Work Permit Instructions

C.W.H.S. students in need of a Work Permit must do the following during this school closure.

Work Permit Process:

1.  Print application from school website.  Print here WORK PERMIT APPLICATION (Students 18 or older do not need a work permit)
2.  Completely fill out part 1 - Student /Applicant Information.  *must be signed by parent.
3.  Have employer fill out part 2 - Pledge of employer.  *must be signed by manager
4.  Must have a current physical form filled out by a doctor. (Good for 1 year from date performed.  If there is a current athletic physical on file, we can use this. If not, Physical Form must be presented.)
5.  After completing steps 1-4, e-mail Mrs. Hummel  [email protected] to come into the building to complete the process .
