Required Documents
To complete your student's registration, we will need the following documents:
- Certified copy of the student's birth certificate
- Proof of Residency (Current mortgage statement or lease agreement AND a current utility bill - gas, water, or electric)
- Custody documents, if applicable
- Special education paperwork, if applicable
- Transcripts or latest grade card, if applicable
- NEW 1st Grade students only: Physician’s Report (date of the exam must be after August 16, 2023). Click here to download a Physical Examination form for your physician to complete.
2024-2025 Kindergarten Students
To complete your student's registration, we will need the following documents:
- Certified copy of the student's birth certificate
- Proof of Residency (Current mortgage statement or lease agreement AND a current utility bill - gas, water, or electric)
- Custody documents, if applicable
- Immunization record
- Physician’s Report (date of the exam must be after August 16, 2023). Click here to download a Physical Examination form for your physician to complete.
Copies will be made of your documents, and the originals will be returned to you. If you do not have access to any of the required documents, please email the registrar or contact the Main Office at (614) 837-4533.
